I think the most likely reason for this, rather than not being paid at all is that a new starter was put on the wrong rate because someone didnt understand the correct rates for different ages. This would mean it's for an even longer period but it could also explain why the employee didn't raise it. A young person in a new minimum wage job is less likely to kick up a fuss and also might not be fully aware of minimum wage laws.

No reputable employer will pay below the minimum wage. Those named in this report are pretty much all admin "errors" due to tax or benefit in kind issues. The problem is with those paid cash in hand or worse, so that HMRC never know about them. The worst culprits are in effect modern slave owners.
