According to my copy of the Rothmans Year Book covering the 94 CC Final, you are correct with Farrell (Cassidy in the 52nd minute).

The other was Panapa for Platt in the 59th minute.

Platt was on the field on 78 minutes.

The player who appears to go missing late on is Dermott.
