Green card use is for when a referee thinks a player is trying to gain an advantage by claiming an injury when it isn't the case.
I believe in the incident last night the referee believed the Wakey player was feigning injury, where as with Minchella and (certainly Lewis) the referee felt there was a genuine injury / cause for the delay.
Normally green card use was related to - or should have related to - feigning / exaggerating injury to slow the game down. Classic case being a defender going down injured when the opposition had just forced a drop out. I'm not sure if there was a directive this year to use it for players claiming to be injured by marginal head contact to try and get a penalty / card from the official.
Clearly last night Lewis did have an injury, can't remember the Minchella one, was it a crusher?
