You can accidentally hit someone high executing a normal tackle which LMS and BMM did. As the disciplinary panel stated about Hill......

Hill basically just hit him and didn't care less where he made contact.

You can accidentally spear tackle someone for example if you miss time it or you can recklessly pick them up and drop them.
